Creating a One-Page Healthcare Profile for Self-Advocates, Family Members and Care Managers

One Page Profiles help people who have healthcare needs to get necessary supports in a way that reflects who they are. One Page Profiles also work to set a baseline of ways in which care can be set up so as to ensure that services and plans are meeting the intended goals.

Whether you work in the healthcare field, are a disabled self-advocate, a parent of a child with special needs, or are caring for a family member with a chronic or disabling condition, come join us for this training to become versed in how One Page Profiles can be of benefit. In this course, you will work on making your own One Page Profile (or one for someone you support) to share with those in the medical field so as to provide an understanding of ways in which to best promote one’s personal health and well-being.

This class is ideal for self-advocates and family caregivers, as well as persons employed as in-home caregivers or staff in the medical field. All will be reminded of person-centeredness of those receiving care.

Attendees will finish this class having created a One Page Profile for use right away.
